The Behavioral Health Specialist provides behavioral health screening, needs assessment, brief interventions, education, care coordination, and referral services as part of Promise Healthcare's integrated care model. The Behavioral Health Specialist works collaboratively with medical, dental, behavioral health, and other care team members to identify behavioral health needs, address barriers to care, connect patients with appropriate services, and support improved health outcomes.

Services may be provided in clinic, school-based, community, mobile, and telehealth settings.

Essential Duties and Responsibilities:

Receives referrals and conducts behavioral health screenings and needs assessments to identify appropriate services and level of care.

Conducts standardized behavioral health screenings and needs assessments using approved tools, which may include PHQ-9, GAD-7, SBIRT/DASH-10, suicide risk screening, and other validated screening instruments appropriate to the patient's age and presenting needs.

Provides brief behavioral health interventions, health education, motivational interviewing, and supportive interventions within the scope of training, credentials, and organizational policies.

Provides warm handoffs to Behavioral Health clinicians and other members of the integrated care team when additional assessment or treatment is indicated.

Provides behavioral health education and resources to patients, families, and groups across the lifespan, including patients age 5 and older, as appropriate to the program.

Collaborates with medical, dental, behavioral health, and social service providers regarding patients' emotional, psychological, physical, and social needs.

Participates in integrated care team meetings, huddles, case consultation, and care coordination activities.

Identifies barriers to care and social drivers of health and assists patients in accessing appropriate healthcare, behavioral health, and community resources.

Coordinates referrals to behavioral health, medical, dental, substance use, social service, and other community resources and follows up as appropriate to support continuity of care.

Advocates for improved social determinants of health and assists patients in navigating available resources.

Supports patient engagement and follow-up for patients who may experience barriers to accessing behavioral health services.

Documents behavioral health screenings, assessments, interventions, referrals, care coordination, patient contacts, and outcomes accurately and timely in the electronic health record, consistent with Promise Healthcare policies and funding requirements.

Maintains scheduling capabilities and supports workflows that promote timely patient access to behavioral health services.

Participates in quality improvement activities, program evaluation, and collection of required behavioral health outcome measures.

Provides services in clinic, school-based, mobile/community, and telehealth settings as assigned.

Participates in emergency and safety procedures and follows organizational protocols for suicide risk, abuse/neglect, crisis situations, and other identified safety concerns.

Maintains current knowledge of local medical, dental, behavioral health, substance use, and social service systems and develops effective working relationships with referral partners.

Maintains required professional licensure, certification, continuing education, and training.

Performs other duties as assigned or required.

EDUCATION / EXPERIENCE REQUIREMENTS:

Master's degree in Social Work or Clinical Counseling pending or waiting for state confirmation of Clinical License (LSW, LPC)
Previous experience working with children, adolescents & families with diverse social, behavioral health challenges.

WORK ENVIRONMENT:

Ability to work in a clinical, school-based, community, mobile, and/or telehealth environment as assigned.

Ability to communicate effectively with patients, families, coworkers, and community partners.
