The Director of Population Health provides high-level leadership in the design, execution, and evaluation of quality improvement (QI) and population health strategies across Promise Healthcare (PHC). This position leads efforts to improve clinical quality, address social determinants of health, and optimize value-based care performance by developing and managing cross-departmental initiatives that enhance access, promote preventive health, strengthen health outcomes, and elevate patient satisfaction across PHC's patient populations.
The Director applies advanced population-level data analytics to guide decision-making, prioritize resources, and evaluate the impact of strategic initiatives. This role also oversees interns and initiatives that support payor incentive alignment, empanelment, care gap closure, and performance-based revenue. The Director plays a critical role in advancing PHC's strategic goals through data-informed population health leadership and QI innovation.
- Lead the vision and strategy for PHC's population health program to support clinical quality outcomes, equity, and financial sustainability per PHC's Strategic Plan.
- Recruit, train, and supervise population health interns and coordinate with other aligned staff responsible for quality and performance-based care coordination.
- Partner with medical, behavioral health, dental, and pharmacy leadership to advance integrated population health strategies while engaging providers in quality and performance improvement through education, feedback, and accountability.
- Develop and maintain relationships with Managed Care Organizations (MCOs) and other payors to optimize incentive programs and value-based agreements.
- Lead advanced analysis of clinical quality, utilization, cost, and social determinant data to identify disparities, forecast trends, and design targeted interventions.
- Work with clinical leaders across service lines to ensure alignment with QI/QA goals.
- Guide population health interventions to close care gaps, enhance preventive screening rates, and improve performance on Uniform Data System (UDS) and payor-based measures.
- Lead value-based care performance by optimizing incentive opportunities, evaluating the financial impact of population health initiatives, and monitoring cost, utilization, and care delivery trends to improve outcomes and sustainability.
- Partner with clinical leadership to ensure continuous feedback from data to frontline teams, supporting agile adjustments and performance accountability.
- Collaborate with OSIS and electronic health record (EHR) staff to define population health metrics and validate data integrity.
- Optimize data extraction and reporting processes to enable real-time performance monitoring and operational decisions.
- Lead QI initiatives using Plan-Do-Study-Act (PDSA) methodology and other evidence-based tools to drive measurable improvements in clinical outcomes and efficiency.
