OverviewThe Hospice Field RN is a professional caregiver who is a member of the interdisciplinary team. Responsibilities include the coordination and provision of direct and indirect patient care using the nursing process to meet the physical, psychosocial, environmental, and spiritual needs of Carle Home Services specific patient populations and families throughout the geographical area. Hours may vary depending upon census and program need.
Responsibilities- Demonstrates understanding of Hospice regulatory requirements and benefits including covered services, recertifications, transfers, and non-recertifications. Follows regulatory documentation standards and completes admission documentation in a timely manner.
- Performs case management activities including assessment, coordination, planning, monitoring, implementation, and evaluation. Interacts with clients, caregivers, and families to assess, plan care, arrange services, monitor, and provide education.
- Ensures physician orders are written timely and corrections to care plans are entered and transmitted according to policy timelines. Verifies orders are not altered after documentation and transmission.
- Reviews verbal orders with the MD office, ensures they are read-back, and transmits within the policy-established time frame.
- Maintains accurate patient visit strings to prevent missed visits and ensures proper use of acronyms on all physician orders.
- Completes Home Health Aide supervisory visits and acts as the coordinator of the health care team to maintain continuum of care via case conferencing and inter-disciplinary communication.
- Communicates pertinent information for patient care conferences on all admits and documents interactions with other disciplines, care coordinators, insurers, and the physician office.
- Provides care according to the plan of care and orders; directs the activities of the licensed practical nurse and assigns and supervises Home Health Aides in the patient home.
- Assesses patients for appropriate Hospice placement per Medicare guidelines and makes home visits to monitor signs and symptoms of end-stage disease and end-of-life conditions.
- Implements symptom palliation interventions in collaboration with attending physicians and triages phone calls when on call.
- Performs home visits to assess and pronounce death, conducts post-mortem care, notifies the County Coroner, funeral home, and physician as appropriate, and provides family support with referrals to Bereavement Coordinator.
- Licensure:
Registered Nurse (RN) in Illinois – IDFPR (current and unrestricted). - Basic Life Support (BLS) Instructor certification within 30 days – American Heart Association.
- Driver's license and Proof of Auto Insurance.
- Education:
College Diploma in Nursing. - Experience:
2 years of nursing experience.
