Overview

$5,000 Sign on Bonus Available (external candidates only)

The Quality Outcomes Coordinator (QOC) works at the department-level, in collaboration with clinical staff and leaders to deliver effective, efficient, high-quality care. The QOC continuously evaluates care and documentation, identifies gaps in processes and facilities process improvement activities to improve outcomes. Implements evidence-based research into practice to ensure patients benefit from the latest innovations in the delivery of care. The QOC serves to increase the driving force and consistency of quality and safety at the unit level.

Qualifications
  • Certifications:

    Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR);
    Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
  • Education:

    College Diploma:
    Nursing
  • Work Experience:

    Nursing
Responsibilities
  • Ability to balance multiple tasks/projects while meeting organizational standards.
  • Utilizes metrics on department dashboards and other quality metrics to prioritize and drive practice changes.
  • Evaluates current practices and implements evidence-based practice recommendations for change.
  • Collaborates with department managers, supervisors, clinical staff, other healthcare providers to address issues related to patient care and outcomes.
  • Participates in work groups to improve patient care issues.
  • Collects data and evaluates trends in patient outcomes in the department (nurse-sensitive indicators, core measures, VBP metrics, occurrence reporting).
  • Reviews overall regulatory compliance by clinical staff.
  • Collaborates with other unit quality outcomes coordinators to improve care across the continuum.
  • Participates in writing and revising policies, procedures, and standards to reflect the latest evidence.
  • Tracks and reports quality outcomes and department trends to management.
  • Provides real-time accountability, coaching, education to promote ownership of safe practice.
  • Utilizes performance improvement methodologies (PDSA) to implement change.
  • Collaborates with department managers, supervisors, other QOCs, and quality department to ensure consistency of practices across the continuum.
  • Reviews enterprise reports, tracks trends, and facilitates process improvement teams to improve processes, promotes transparency of data and accountability by providing quality reports to department leaders and clinical staff on a daily, weekly, monthly, quarterly, and yearly basis.
  • Collaborates with leadership and educators in conducting a needs assessment for continued education.

Compensation and Benefits

The compensation range for this position is $37.16 per hour - $63.92 per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered to a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
