We proudly serve patients in their homes across Champaign, Vermilion, Edgar, Douglas, Moultrie, and Macon counties.
Schedule Highlights- Limited on call and weekend shifts
- Consistent weekday schedule Monday‑Friday 8am‑4:30pm
- Provide on‑call coverage and perform patient visits as assigned for hospice care.
- Conduct telephone triage, visits, and admissions when hospice office is closed.
- Maintain clinical records and documentation with accuracy.
- Communicate hospice services and end‑of‑life treatment choices to patients, families, and health care providers.
- Assess referrals for medical, psychosocial, and environmental appropriateness and obtain certification from attending physician.
- Coordinate care and medication delivery, durable equipment, and supplies.
- Obtain informed consents and complete admissions in home, nursing home, and hospital settings.
- Follow protocols for symptom management, patient visits, and admission procedures.
- Participate in QA/CQI processes as assigned.
- Licensed Registered Professional Nurse (RN) – Illinois Department of Financial and Professional Regulation (IDFPR).
- Proof of Auto Insurance (varies).
- Basic Life Support (BLS) within 30 days – American Heart Association (AHA).
- Driver's License – Illinois Secretary of State (ILSOS).
- Associate’s Degree.
- 1 year of nursing experience.
Hourly rate: $33.62 – $57.83 per hour. Salary depends on experience, qualifications, location, training, licenses, shifts, etc. The benefits package includes comprehensive coverage; see careers.carlehealth.org/benefits.
