Day Shift Nurse - Heartis Village of Peoria

Heartis Village of Peoria, under Discovery Management Group, is hiring a Day shift Licensed Practical Nurse (LPN) to provide compassionate, high quality nursing care to seniors. The role requires hands-on nursing care according to individualized care plans, monitoring resident conditions, and collaborating with interdisciplinary teams.

• Status: Full time or part time
• Schedule: 6 am - 6 pm, including every other weekend
• Location: 8201 N. IL Route 91, Peoria, IL 61615
• Rate of Pay: $30.61 - $34.51 per hour, based on experience

• Provide nursing care to residents per care plans, policies, and procedures.
• Conduct ongoing assessments of residents’ physical and psychosocial status.
• Administer medications and treatments as prescribed and document appropriately.
• Coordinate and communicate resident care needs with physicians, families, pharmacies, and other care providers.
• Support health promotion, disease prevention, and wellness initiatives within the community.
• Ensure accurate, timely, and compliant clinical documentation.
• Assist with care planning, service plan updates, and resident evaluations as required.
• Collaborate with caregivers, wellness team members, and leadership to ensure consistent, high-quality care delivery.
• Respond to resident concerns, incidents, and emergencies using sound clinical judgment.
• Support training and guidance for care staff as appropriate.
• Ensure the community adheres to applicable federal, state, and local regulations related to resident care and services.
• Participate in quality assurance initiatives, audits, and compliance activities as needed.
• Perform other duties as assigned.

• Current Licensed Practical Nurse (LPN) license, or a Professional Registered Nurse license as per state regulations.
• Minimum of two (2) years of licensed nursing experience.
• Experience in assisted living, long-term care, or a skilled nursing environment preferred.
• Strong clinical judgment, problem-solving, and decision-making skills.
• Effective organizational, time-management, and communication skills.
• Ability to manage multiple priorities and delegate appropriately.
• Demonstrated commitment to seniors and person-centered care.
• Ability to work nights, weekends, and flexible schedules as required by the community.
• Proficiency with electronic documentation systems and basic computer applications.
• Ability to respond to medical emergencies.
• Exposure to blood borne pathogens and infectious disease precautions.
• Ability to lift or assist residents as required by care plans.
