Job Description
Be a part of a world‑class academic healthcare system, UChicago Medicine, as a Nurse Navigator in Hyde Park. This is a 100% onsite position; you must be based in the greater Chicagoland area. Under the direction of a faculty sponsor and in collaboration with the program support team, the Nurse Navigator will coordinate the care of patients who have recently undergone a heart transplant.
The Nurse Navigator will act as a clinical liaison for patients across medical services and clinical departments, assessing clinical, emotional, spiritual, psychosocial, financial, and other patient needs and evaluating and managing patient symptoms before, during, and after treatment.
- Provides care coordination and patient education to patients in the service line.
- Supports discharge planning for the service line.
- Facilitates resolution to patient care gaps and monitors follow‑up/completion by proactively reaching out to patients and addressing direct phone calls and messages.
- Acts as liaison for patients, families and their multidisciplinary clinical team.
- Educates and reinforces education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on the multidisciplinary care plan.
- Clinical documentation review to improve length of stay.
- Develops and/or coordinates patient education materials, clinical pathways, and quality studies.
- Serves as primary point of contact for patients discharged from the hospital to address questions and patient care needs to prevent readmission or an emergency room visit.
- Actively licensed Registered Nurse in the state of Illinois.
- Minimum of two years clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients.
- General computer skills and adaptability to changing working situations and assignments.
- Excellent interpersonal skills with the ability to communicate effectively with patients, family members, and physicians.
- Certified Nurse in Specialty area or in process.

The pay range is based on a full‑time equivalent (1.0 FTE) and is reflective of current market data, reviewed annually. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity.
