LPN - Convenient Care

Overview

The Licensed Practical Nurse (LPN) assumes responsibilities for direct nursing care of assigned patients under the supervision of a registered nurse (RN) or other provider in patient care departments. Provides nursing services to patients and families in accordance with the LPN scope of practice as defined by the Illinois Board of Nursing.

• Licensed Practical Nurse (LPN) with current Illinois Department of Financial and Professional Regulation (IDFPR) license
• Basic Life Support (BLS) certification within 30 days of employment with American Heart Association (AHA)
• College Diploma or Certificate of Completion in Licensed Practical Nursing (required)
• Relevant work experience in nursing (minimum)

• Provide direct nursing care to patients under the supervision of a registered nurse, midlevel provider, or physician.
• Follow provider orders and the nursing plan of care within the LPN scope of practice.
• Report patient responses to care to the registered nurse or other provider for evaluation, intervention, and modification of the plan of care.
• Ensure documentation of care is complete and accurate.
• Administer medications orally, rectally, intramuscularly, and via inhalation.
• Assist providers with procedures such as vaginal exams, suturing, staple placement, incision and drainage.
• Member of the Code Team.
• Work in basket result notes.
• Serve as preceptor for new staff.
• Clean and prepare instruments for sterilization.
• Room patients: collect pertinent information regarding chief complaint, vital signs, and medication reconciliation upon arrival.
• Conduct telephone interactions with patients, families, pharmacies, and other health care facilities per department policies and use approved Telephone Triage books under supervision of a registered nurse or provider.
• Perform office procedures and procedures such as suturing, staple removal, split‑splint application or removal, crutch training, catheterization, IV hydration therapy, and read TB skin test results as directed.
• Provide patients with written instructions regarding procedures, medications, or follow‑up instructions per provider direction.
• Assist with prescription medication refills per department protocols: call pharmacies, notify patients of refills, and document actions within the EMR per provider orders.
• Maintain department‑specific competencies related to point‑of‑care testing, equipment handling, and specialty training.
• Obtain vitals and collect data for patients to be seen by provider.
• Perform waiting‑room and exam‑room rounding.
• Collect and process specimens correctly.
