Travel Skilled Nursing Facility/Rehabilitation Nurse

Join a 13-week SNF/rehab travel assignment in Champaign, IL. Provide skilled nursing care to older adults with complex needs in a skilled nursing facility or rehab setting. Collaborate with interdisciplinary teams, document patient care, administer medications, and monitor patient status to support optimal outcomes.

Key Responsibilities:
  • Provide skilled nursing care for SNF/Rehab patients
  • Collaborate with interdisciplinary teams to support patient care plans
  • Maintain accurate, timely documentation of assessments, interventions, and progress
  • Execute medication administration and treatment procedures safely
  • Monitor patient condition changes and communicate updates promptly
  • Handle multiple patient assignments in a fast-paced environment
  • Work scheduled weekend days (Friday, Saturday, Sunday)
  • Qualifications:
    • Graduate of an accredited nursing program with an RN degree (ADN or BSN)
    • Current RN licensure in Illinois (or compact eligibility)
    • Current BLS certification; ACLS preferred
    • Minimum 1 year of experience in SNF/Rehab or long-term care
    • Experience providing direct patient care for older adult populations with complex care needs
    • Experience charting/documenting nursing care in an electronic or paper clinical documentation system
    • Strong clinical assessment and communication skills for interdisciplinary care coordination
    • Ability to work scheduled weekends (Friday, Saturday, Sunday)
    Preferred

    Skills:
    • ACLS certification (preferred)
