Overview
Position requires 6 months of in-office training prior to being remote. The office is located in Champaign, IL.
$5,000 sign‑on bonus - external candidate only.
The Patient Advisory team is the first point of contact for our immediate and extended communities. PAN RNs are key to ensuring our patients are cared for 24/7. By providing the ideal blend of autonomy of practice and nursing judgement while still offering support and protections through outlined processes, the ideal candidate will enhance our talented team by offering mastery in chronic and acute processes, strong verbal communication skills, critical thinking, and the ability to thrive in a fast‑paced environment, while remaining patient‑centric.
Qualifications- Licensed Registered Professional Nurse (RN) – Indiana (IN), within 1 year, licensed by Indiana Professional Licensing Agency (IPLA).
- Licensed Registered Professional Nurse (RN) – Illinois (IL), within 1 year, licensed by Illinois Department of Financial and Professional Regulation.
- Licensed Registered Professional Nurse (RN) – Washington (WA), within 1 year, licensed by Washington State Board of Nursing.
- Licensed Registered Professional Nurse (RN) – Ohio (OH), within 1 year, licensed by Ohio Board of Nursing.
- Licensed Registered Professional Nurse (RN) – Iowa (IA), within 1 year, licensed by Iowa Board of Nursing.
- Education:
Diploma in Nursing. - Work Experience:
Nursing.
- Assesses and triages patient concerns via telephone/video/electronic methods using embedded adult and pediatric protocols.
- Assesses health needs, develops plans of care, and coordinates care for patients with health concerns.
- Documents all telephone encounters within Epic and completes appropriate EMR documentation and follow‑up.
- Provides medication management assistance by renewing, refilling, cancelling, transcribing, and reconciling orders within departmental guidelines.
- Ensures adherence to established standards of care and scope of practice.
- Provides education and counseling to patients, families, and groups.
- Facilitates care coordination between patient and provider during 24/7 operations.
- Communicates with outside facilities when appropriate.
- Assists with department tracking, auditing, and reporting for improvement opportunities.
- Serves as a resource within the organization and the community, providing additional support to other Carle clinical team members and patients when needed.
- Act as preceptor and supports the development and retention of other staff.
- Recognizes and initiates change initiatives that improve clinical outcomes using evidence‑based practices.
- Provides leadership for initiatives to improve quality of care or clinical outcomes.
- Assists leadership team in developing and implementing departmental processes.
- Primarily provides telehealth triage for acute needs for Carle Health and contracted health systems through daytime and after‑hours to facilitate and maintain patient safety.
- Provides after‑hour care of patient needs, supports critical patient communications, and maintains working knowledge of specialized processes.
- Assumes essential Charge RN functions in rotation with other team members, directly supporting immediate team needs, operational flows, and patient care support.
- Cross‑trains to all Patient Contact Center departments to assist with patient care needs requiring medication refills, telephone triage, and patient portal communications.
The compensation range for this position is $33.11 per hour - $53.88 per hour. This range reflects a good‑faith minimum and maximum for the role at the time of posting. Actual compensation offered to a candidate will depend on factors including experience, qualifications, location, training, licenses, shifts, and compensation model.
