Position: RN - Hospice (Champaign Region)

Overview

Sign on Bonus: $7,500 for candidates with more than 1 year of experience.

Relocation Bonus available for moves greater than 50 miles (external candidates only).

We proudly serve patients in home settings across Champaign, Vermilion, Edgar, Douglas, Moultrie, and Macon counties.

    Full‑time: $4,000 per calendar year;
    Part‑time: $2,000 per calendar year.

Responsibilities
  • Coordinate and provide direct and indirect patient care using the nursing process to meet physical, psychosocial, environmental, and spiritual needs.
  • Follow regulatory requirements and documentation standards; complete admission, recertification, and visit notes in a timely manner.
  • Assess, plan, coordinate, and evaluate patients' care; communicate with clients, caregivers, families, and multidisciplinary team.
  • Act as coordinator of the healthcare team, maintain proper linkages, and document case conferences.
  • Supervise licensed practical nurses and home health aides; assign care activities and provide instructions.
  • Implement interventions to palliate symptoms, perform home visits, and provide end‑of‑life care in accordance with Medicare guidelines.
  • Triage phone calls, make home visits to assess and pronounce death, notify County Coroner and funeral home, and refer families to bereavement services.
Qualifications
  • Licensed Registered Professional Nurse (RN) – Illinois Department of Financial and Professional Regulation.
  • Basic Life Support (BLS) Instructor within 30 days – American Heart Association.
  • Valid driver's license – Illinois Secretary of State.
  • Proof of auto insurance.
  • College diploma in Nursing.
  • Minimum 1 year of nursing experience.
Compensation

Hourly rate ranges from $35.87 to $58.37 per hour, dependent on experience, qualifications, location, and other factors.
