ResponsibilitiesOur Case Managers use a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost‑effective outcomes.
- Act as liaison with members, employers, providers, insurance companies and other stakeholders.
- Implement and coordinate all case‑management activities for catastrophic and chronically ill members/clients across the continuum of care, including referrals, home‑care visits, community resources and alternative levels of care.
- Conduct assessments and analyze medical and vocational status to develop a plan of care that facilitates optimal wellness and medical outcomes, and helps members return to work or function at baseline.
- Communicate with members and stakeholders telephonically or in person; maintain documentation of case‑work activities.
- Consult with internal multidisciplinary teams and outreach to treating physicians or specialists as appropriate.
- Provide education, prevention information and support for best medical outcomes.
- Apply all relevant laws, regulations and company policies to rehabilitation services and benefit eligibility.
- Monitor member progress toward desired outcomes through assessment and evaluation.
- Must reside in Rock Island, Mercer, Whiteside or surrounding counties.
- Ability to travel up to 50–75% of time within the designated geographic area.
- Active and unrestricted Illinois Registered Nurse (RN) license.
- 3–5 years clinical practical experience.
- Confidence working independently and with virtual collaboration tools.
- Excellent analytical and problem‑solving skills.
- Effective communication, organizational and interpersonal skills.
- Ability to work independently.
- Proficiency with Microsoft Word, Excel, Outlook, PowerPoint and proprietary applications.
- Certified Case Manager (CCM).
- 2–3 years care‑management, discharge‑planning and/or home‑health care coordination experience.
- Associate’s Degree in Nursing (REQUIRED).
- Bachelor’s Degree in Nursing (PREFERRED).
- Active and unrestricted Illinois Registered Nurse (RN) license.
Anticipated weekly hours: 40 hours. Full‑time Position. Pay range: $66,575.00 – $ annually, inclusive of base salary, bonus and incentive programs.
