As an In-House Nurse Practitioner specializing in Infectious Disease, you will be the primary provider helping your facilities clinical expertise in their Infectious Disease programs by participating in appropriate resident's plan of care. You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

Job Requirements:
Active Illinois Nurse Practitioner License (unencumbered).
Master's Degree in Nursing from an accredited program.
Strong clinical background in Infectious disease.
Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.
Key Responsibilities:
Performing comprehensive physical examinations and evaluating changes in patient conditions
Prescribing medications and treatments as needed
Developing patient care policies and facilitating referrals to specialists
Coordinating advanced care planning and discharge visits
Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups
Supporting the physician-patient relationship and addressing gaps in care
Our NPs work closely with the interdisciplinary team, taking a leadership role in patient care, collaborating with physicians, and ensuring high-quality service delivery in our facilities.
Salary & Compensation:
Annual Pay
: $105,000 - $130,000 based on experienceMonthly Bonus Opportunities
