RN Case Manager - Home Health

We are seeking an experienced Home Care RN/Field Case Manager to provide skilled nursing visits and coordinate patient care in a community-based setting.

• Responsibilities include:
  
  assessing patients, developing and managing care plans, performing complex wound care, managing central lines and PICC lines, administering infusions, and monitoring devices such as catheters, drains, and tracheostomies.
• Educating patients and families to promote health, independence, and wellness.
• Collaborating with an interdisciplinary team, directing licensed practical nurses and home health aides, and ensuring regulatory compliance in documentation and care standards.
• Conducting OASIS assessments, managing case documentation, and communicating with physicians and care team members regarding patient status and updates.

• Valid IL RN license, BLS certification, and a valid driver’s license with proof of auto insurance.
• Minimum of 1 year nursing experience.
• Knowledge of Medicare home care benefits, regulatory requirements, and documentation standards.
• Strong assessment, communication, and organizational skills with the ability to work independently.
