Temporary Value-Based Care Registered Nurse (North Shore)
Position Summary:
Now hiring hybrid nurses to visit our patients in the North Shore area of Chicago! Serves on the dynamic care team and delivers personalized care to patients within their homes. Plays a crucial role in assessing clinical and environmental needs, fostering a healing environment, and supporting patients and their families through education and emotional support.
Primary ResponsibilitiesProvides high-quality care management and acute care to patients enrolled in the Primarily Home program. Conducts in-home comprehensive assessments and develop individualized care plans tailored to each patient’s needs. Supports an overall healing environment by spending additional time with patients in-home assessing clinical and environmental needs. Acts as a key educator and provides ongoing compassionate and empathetic support to patients and their families, ensuring they have the necessary resources and understanding to manage their care in the comfort of the home.
Collaborates closely with interdisciplinary teams to coordinate care and optimize patient outcomes.
Associate's degree combined with 3+ years of relevant experience or the knowledge, skill, or ability to succeed in the role. Registered nurse in good standing in the state of Illinois and eligibility for licensure in Missouri Must have or obtain Basic Life Support (BLS) within 30 days of hire. Valid Driver’s license, automobile insurance, and access to reliable transportation to visit patients.
Preferred QualificationsBachelor's degree from an accredited program with a major in nursing Acute and home care experience a plus Registered nurse in good standing in the state of Missouri
Working ConditionsWhile performing the duties of this job, the employee will be expected to drive a vehicle, transfer patients, lift or carry medical supplies, operate complex and delicate equipment with precision and accuracy, operate a personal computer, and frequently interact with care providers and patients

Factors that may be used to determine your actual pay rate include your specific skills, experience, qualifications, location, and comparison to the other employees already in this role. In addition to the base salary, certain roles may qualify for a performance-based incentive and/or equity, with eligibility depending on the position. These rewards are based on a combination of company performance and individual achievements.
The hiring range for this position is: $72,800.00-$97,400.00
