Overview

The Home Services Social Worker identifies psychosocial needs of patients and families through assessment. Social work interventions range from resource support identification and acquisition (including community support, financial and environmental enhancement) to short‑term counseling and emergent crisis intervention. Social services are provided as part of collaboration with interdisciplinary teams. Hours may vary depending upon census and program need.

Qualifications
  • Licensed Clinical Social Worker (LCSW) – Illinois Department of Financial and Professional Regulation (IDFPR)
  • Licensed Social Worker (LSW) – IDFPR
  • Driver’s License – Illinois Secretary of State (ILSOS)
  • Proof of Auto Insurance – Varies
  • Education:

    Master’s Degree in Social Work (Required)

Responsibilities
  • Provide psychosocial assessments of patients and families to identify strengths and problems related to diagnosis, illness, treatment, and/or life situation.
  • Develop a bereavement plan of care to address family members’ and caregivers’ needs.
  • Interpret and communicate patient/family faith and cultural traditions, and identify when spiritual/religious beliefs impact physical and psychosocial care.
  • Educate patient and family members in a manner that overcomes barriers, matches learning capabilities, and meets fundamental needs.
  • Complete visit notes, orders, and corrections to care plans in a timely manner according to policy.
  • Document psychosocial patient/family assessments, financial assessment, and MSW interventions within the electronic medical record.
  • Implement social work plan that enhances family system strength, utilization of community resources, maximization of medical benefits, environment for care delivery, dignity for the dying patient, patient coping skills, and support for cultural beliefs and values.
  • Evaluate effectiveness of social work plan of care and modify interventions as indicated.
  • Facilitate and support patient decisions and communication of self‑determined life care decisions.
  • Provide care according to plan of care/orders and collaborate in developing the social work plan of care with IDT.
  • Practice sensitive to needs of patients and families, focusing on palliative/comfort philosophy.
  • Identify and respond to indicators of imminent death, addressing patient/family needs at time of death.
  • Demonstrate understanding of Medicare Hospice Benefit, including eligibility, qualification, election process, certification, recertification, transfer, non‑recertification, and revocation.
  • Utilize Memorial Funds appropriately and submit documentation according to policy.
  • Complete or assist patients/caregivers in advanced directives, including living will, HCPOA, and POLST forms.
  • Assist with transportation barriers, referrals for lack of access to food, clothing, assistance with power bills, help in the home, application assistance for Medicaid, Community Care, SSDI, and make Elder Abuse/Neglect referrals.
Compensation and Benefits

The compensation range for this position is $27.36 per hour – $45.69 per hour. This represents a good‑faith minimum and maximum range for the role at the time of posting. The actual compensation offered to a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
