Designated Coordinator/Services Manager

This is a crisis group home which is a short-term, highly supportive residential setting designed for children, teens, or adults who are experiencing an emotional, behavioral, or other crisis. The goal is stabilization—not long-term placement—so the person can return home or move to a more appropriate setting once the individual has stabilized and a plan is in place. The individuals we support in this setting are often highly behavioral, may be violent at times, display verbal aggression, and may elope.

It is imperative that employees following our training, policies, and care plans to support individuals with these unique needs.

Designated Coordinators/Service Managers provide leadership and management to assigned group home operations. It is the responsibility of this individual to ensure the site provides a respectful, inclusive, caring, and supportive environment. This position also functions as a point of contact for families, team members, and individuals served.

Designated Coordinators/Service Managers get the opportunity to teach House Supervisors and Direct Support Professionals about how to be successful in this field! The responsibilities of a Designated Coordinator/Services Manager include:

• Overseeing assigned programs and ensuring all licensing regulations and requirements are met.
• Reviewing and ensuring individual team meeting paperwork is prepared and accurate. Ensuring maintenance of individual's records and performing routine audits.
• Coordinating with the individuals and their teams to develop person-centered approaches to identify what is important for each individual.
• Managing financial resources including general budgets, payroll budgets, program accounts, and petty cash accounts.
• Ensuring the implementation of all health-related directives from health care professionals, ensuring the Registered Nurse is kept informed about individual health service delivery and health-related incidents.
• Ensuring GERs, Emergency Use of Manual Restraint Forms, Behavior Intervention Reporting Forms, and Serious Injury Reporting Forms are completed according to policy.
• In coordination with Residential Director, evaluating program outcomes and participating in quality improvement projects.
• Acting as a liaison between the company and schools, hospitals, courts, probation officers, guardians, family members, health providers, case managers, and other team members.
• Delivering direct care services and supervision during staffing shortages.

Applicants must:

• Have three to five years of experience in a program that provides direct care or education to vulnerable adults or children.
• Possess a valid driver's license, have access to an insured vehicle, have a clean driving record, and need to pass a DHS background study.
• Have a proven history of developing staff and managers.
• Working knowledge of 245D requirements and standards.
• Exhibit proficiency in all Microsoft office applications to be successful in this position. Therap experience a plus.

Degree and Experience Requirements:

• (1) A baccalaureate degree in a field related to human services, education, or health and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children; OR
