Sevita is seeking a compassionate Group Home Caregiver to support individuals in a community-based residential setting. In this role, you will assist with daily living activities, personal care, medication reminders, transportation, and community integration, while promoting dignity, independence, and safety. You'll implement care plans, document services, and collaborate with healthcare professionals and families. Responsibilities

  • Provide direct care and support with activities of daily living (bathing, dressing, grooming, toileting, mobility)
  • Assist with meal planning, preparation, feeding, and adherence to dietary guidelines
  • Administer or assist with medications per policy and document accurately
  • Monitor and report changes in physical, emotional, or behavioral status
  • Implement individual service and behavior support plans as directed by clinical staff
  • Support individuals in community outings, appointments, and recreational activities
  • Encourage independence and skill-building in household and self-care tasks
  • Maintain a clean, safe, and stable group home environment
  • Follow safety, infection control, and emergency procedures at all times
  • Communicate effectively with supervisors, team members, families, and guardians
  • Complete required documentation and incident reports in a timely manner
