Job Summary

Position Details
  • 0.8 FTE (64 hours per two‑week pay period)
  • Shift: Day/Evening, 8‑hour day or evening shifts, no weekends
  • Floating may be required (eligible for mileage reimbursement)
Responsibilities
  • Conduct comprehensive assessment of patients and families in an outpatient setting.
  • Analyze assessment data to determine nursing diagnoses and validate them with the patient, family and health‑care team.
  • Identify expected outcomes and collaborate with patients and families to set realistic, measurable goals.
  • Develop individualized plans of care that incorporate evidence‑based interventions and consideration of statutes, rules and regulations.
  • Implement and document interventions safely and timely, coordinating with the continuum of care as needed.
  • Evaluate patient progress, revise diagnoses, outcomes and plans, and document changes according to guidelines.
  • Participate in quality improvement activities and utilize QI data to enhance nursing practice.
  • Engage in continuous learning through educational activities and apply new knowledge to practice.
  • Critically evaluate one’s own practice relative to professional standards and seek feedback from patients, peers and colleagues.
  • Provide collegial support by sharing knowledge, giving constructive feedback and fostering a positive work environment.
  • Collaborate with patients, families and interdisciplinary teams to achieve optimal outcomes.
  • Act ethically, maintain professional boundaries and advocate for patients, assisting them in developing self‑advocacy skills.
  • Leverage research findings to inform practice decisions and integrate evidence into care.
  • Manage resources effectively, ensuring safe and cost‑effective patient care.
  • Demonstrate leadership as a professional role model, promote shared decision‑making, and support a healthy workplace.
Charge Nurse/Preceptor Duties (when acting in these roles)
  • Coordinate unit operations to meet patient and family needs efficiently.
  • Support growth of staff, foster teamwork, and serve as a professional role model.
  • Develop and implement individualized orientation plans, assess competence, and facilitate professional development of new hires.
Required Qualifications
  • Associate's or vocational degree graduate of an accredited school of nursing.
  • Registered Nurse – MN Board of Nursing license required if working in MN; RN license for WI required if working in WI.
  • BLS Tier 1 (Basic Life Support) required within 30 days of hire.
  • Minimum of 0 to 2 years of RN experience; new graduate RN positions considered.
  • Preferred:
    Bachelor’s degree graduate of an accredited nursing program.
Preferred Qualifications
  • RN‑BC Ambulatory Care certification.
Physical Demands

Medium effort. Employees may need to lift up to 32 lbs. occasionally and 25 lbs. frequently.

Pay Range

$40.08 to $54.93 per hour – base hiring rate varies based on experience, education and applicable agreements.
