The Certified Nurse Practitioner is a member of the primary health care team who works in collaboration with physicians and other medical staff to provide comprehensive health care to patients. The Nurse Practitioner engages in independent decision making about health care needs and provides health care services to individuals and families. The Nurse Practitioner obtains patient histories, performs physical and psychological health assessments, provides patient and family education and counseling, ensures continuity of health care services, and manages acute and chronic disease conditions.

The Certified Nurse Practitioner reports directly to the Chief Executive Officer.

• Must be at least 18 years of age.
• Proof of highest level of education completed but not less than high school education or equivalent.
• Graduate of or successful completion of an accredited nurse practitioner training program and at least a Master’s degree with a concentration in the requested specialty.
• Must have current, active Arizona RN and AP licensure and be eligible for privileges to dispense and prescribe medications. Must be eligible for DEA registration.
• Must meet threshold criteria/qualifications for hospital privileges and membership on the Hospital Medical Staff. Must be eligible for credentialing with individual payers.
• Minimum of three (3) years of experience as a Nurse Practitioner in a clinic setting with additional nursing experience preferred. Special considerations may be made based on clinic need and patient demand at the discretion of administrator.
• Current Arizona Department of Public Safety fingerprint clearance card (new hires must also complete the application process for this card before being released to their department).
• Maintains current CPR Certification from the American Heart Association (Healthcare Provider) or other approved educational programs.
• Participation in clinic/organizational committees as requested. Must have computer and keyboarding skills, demonstrated leadership, teaching, evaluation and communication skills.
• Ability to effectively communicate in oral and written form.
• An understanding of computer systems, process design and data management.
• Must have a well‑groomed, professional appearance.
• Must have the mental and physical ability to perform, with or without reasonable accommodation, the essential functions of the job.
• Ability to lift, carry, pull, & push 50 pounds.
• Language proficiency in English, with excellent written and verbal fluency.

• Previous Meditech experience or other related electronic records systems.
