• Within Case Management, the RN Case Manager will guide a comprehensive care coordination, utilization review, and discharge planning program that excels at safe transitions of care, denials prevention, and patient advocacy across the continuum.

• Reporting to a Director of Case Management or an executive over Population Health, the Nurse Manager of Case Management will have operational and leadership oversight for RN Case Manager teams, social workers, and support staff, with access to physician advisors, data analysts, clinical documentation specialists, and a dedicated educator to enhance competencies and compliance.
• The RN Case Manager leader will drive daily huddles, round with interdisciplinary partners, collaborate on complex discharges, steward regulatory readiness, strengthen payer partnerships, refine order sets and protocols, and coach a friendly, cohesive, and diverse team of nurses and allied professionals who value respect, empathy, and shared wins.
