Position Summary

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and standards of professional nursing practice. It is accountable for the quality of nursing services delivered by self or others under their direction and utilizes specialized knowledge, judgment, and nursing skills to provide care appropriate to the physical and developmental age of assigned patients.

Functions

Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and ensures completion within appropriate timeframe and policy requirements.

Formulates a plan of care, including the discharge plan, based on assessment data and input from patients, families, and the health team. Sets realistic, measurable goals and revises plans as patient needs change.

Implements the plan of care through direct patient care, coordination, delegation, and supervision of the health care team, ensuring compliance with policies, procedures and regulatory requirements.

Evaluates patient and family response to care and teaching, revising the plan of care as needed and collaborating with the health care team on patient progress toward goals.

Documents assessment, planning, implementation and evaluation in the patient record in a legible, timely manner according to policy.

Provides evidence‑based care, participates in research activities, and engages in unit or facility shared leadership. Supports others through precepting and mentoring.

Contributes to excellent patient outcomes in a timely, effective, efficient, equitable, and safe manner, and participates in the improvement of national nursing quality indicators.

In some roles, may supervise staff and workflow of the department.

Interacts with all levels of staff, physicians, patients, families and external contacts, synthesizing data from multiple sources to support responses to changes in patient health status.

Minimum Qualifications

Current, valid RN license in state of practice, temporary/interim RN license, or compact RN licensure. BLS certification required; additional certifications may be required based on area of practice.
Desirable experience in the clinical area applied for. Must maintain clinical performance competencies through annual validations. One year of acute or physician practice experience in an acute care setting within the past 12 months is required for certain positions.
Home Health candidates must operate a personal vehicle, possess a valid driver’s license, and maintain active auto insurance.
Behavioral Health staff must possess an Arizona Fingerprint Clearance Card at hire and maintain it throughout employment.
Bachelor’s degree preferred. Professional certification preferred. Additional related education and/or experience preferred.

Benefits and Compensation

Full Time, Night Shift with three 12‑hour shifts per week. Weekend and holiday rotations required. Flat rate $3.00/hour weekend shift differential and $3.00/hour float incentive. Opportunity to participate in a variety of health, financial and security benefits.
