Licensed Practical Nurse

Life Care Centers is seeking a compassionate LPN Licensed Practical Nurse to join our Healthcare & Medical Services team. In this on-site role, you will provide direct bedside care to senior residents, administer medications and treatments, monitor clinical status, and accurately document in the EHR. You will collaborate with an interdisciplinary team to deliver patient-first, family-focused care, support care planning, and educate residents and families.

Responsibilities

• Provide direct bedside nursing care to senior residents in a skilled nursing and rehabilitation setting
• Administer medications and treatments according to provider orders and facility policies
• Monitor, document, and report changes in resident condition using EHR systems
• Support and implement individualized care plans with the interdisciplinary care team
• Perform wound care, catheter care, and basic respiratory support per protocol
• Educate residents and families on care plans, medications, and health management
• Maintain strict infection control, safety, and regulatory compliance standards
• Precept and support new staff, CNAs, and team members as needed
• Participate in shift report, care conferences, and quality improvement initiatives
