Field Nurse Practitioner

Job Description

Under the supervision of the Vice President, Clinical Services, the Field Nurse Practitioner provides direct patient care to members in a variety of settings to improve quality outcomes and patient experience while helping mitigate resource utilization in support of organizational goals. The NP practices autonomously with an emphasis on disease prevention, health management, and effective care for acute and chronic conditions.

Responsibilities

• Provide quality patient care within the nurse practitioner scope of practice and established regulatory, licensing, and evidence-based standards.
• Perform annual wellness visits in private residences, assisted living facilities, provider and specialist offices, hospitals, and skilled nursing facilities.
• Conduct chart reviews to support accurate diagnoses, patient status, and individualized plans of care.
• Assess patients for acute and chronic conditions and diagnose conditions affecting care and treatment.
• Complete accurate, timely documentation of assessed conditions in accordance with established guidelines.
• Demonstrate proficiency in Risk Adjustment Factor and Hierarchical Condition Categories model measures within 90 days of hire.
• Demonstrate proficiency in Quality STAR and HEDIS measures within 90 days of hire.

Required Qualifications

• Master’s degree in Nursing.
• Board certification as an Advanced Practice Nurse from a nationally recognized certifying body, as required by the Arizona State Board of Nursing.
• Family Nurse Practitioner or Adult Nurse Practitioner certification.
• Current unrestricted Arizona RN license.
• Current Arizona Advanced Practice Nursing license; a temporary license may be accepted pending national board certification and receipt of permanent licensure.
• Full unrestricted prescriptive and dispensing privileges with DEA licensure.
• Current basic CPR certification or ability to obtain certification within 30 days of hire.
• Willingness and ability to travel throughout the valley.
• Self-direction and excellent organizational skills.
• Excellent written and verbal interpersonal communication skills.
• Current Arizona driver’s license and proof of insurance.

Preferred Qualifications

• Geriatric experience.
