Field- RN Care Manager — CKD & Home Care

Somatus in Chandler, AZ is seeking an RN Care Manager, VBC to coordinate care for high-need CKD/ESKD patients across home, clinic, and facility settings. You will partner with nephrologists, PCPs, and interdisciplinary teams to address barriers, educate patients, and implement individualized care plans that improve outcomes.

Travel within the assigned region is required as you support care coordination, conduct home visits, and participate in regular care team meetings.
