Community Care Liaison

• Community Care Liaison is a key position within our home care agency
• Primarily work in the field assessing clients, mentoring caregivers and building referral relationships
• Educate healthcare providers, community organizations, and families about care services
• Ensure a smooth transition for seniors and individuals with disabilities who need daily living assistance
• Responsible for outside sales, relationship building, and community outreach
• Cultivate a robust referral pipeline while actively increasing market awareness
• Work in a team environment and reports directly to the owner

• Must have 3 or more years of proven experience in social work, case management, home health, hospice AND/OR home care
• Compassionate, kind, caring and be a great listener
• Outstanding problem-solving skills and ability to work in a fast-paced business
• Understanding of working in a continuum of care industry
• Excellent follow through skills and self-starter
• Able to pass series of background checks, Level One Fingerprint Clearance Card, Proof of Negative TB, Current First Aid and CPR Card
• Experience in the hospice B2B referral building field is a plus!

Demonstrates expertise in community outreach, relationship building, and client assessment within the home care industry. Proficient in mentoring caregivers and educating stakeholders about care services while ensuring smooth transitions for clients requiring daily living assistance.
