What You'll Do
  • Provide evening hospice visits to patients throughout the Twin Cities Metro.
  • Perform assessments, symptom management, medication administration, and comfort-focused nursing care.
  • Educate and support patients, families, and caregivers.
  • Deliver care in homes, assisted living communities, memory care, and skilled nursing facilities.
  • Coordinate closely with physicians and the interdisciplinary hospice team.
  • Complete timely and accurate EMR documentation.
What You'll Bring
  • Active Minnesota Registered Nurse (RN) license.
  • Hospice experience is welcome, but nurses from ICU, ER, oncology, med-surg, home health, telemetry, step-down, and palliative care are encouraged to apply.
  • Strong clinical assessment and communication skills.
  • Confidence working independently in the field.
  • A passion for building meaningful relationships through exceptional patient care.
