What You'll Do
- Provide evening hospice visits to patients throughout the Twin Cities Metro.
- Perform assessments, symptom management, medication administration, and comfort-focused nursing care.
- Educate and support patients, families, and caregivers.
- Deliver care in homes, assisted living communities, memory care, and skilled nursing facilities.
- Coordinate closely with physicians and the interdisciplinary hospice team.
- Complete timely and accurate EMR documentation.
What You'll Bring
- Active Minnesota Registered Nurse (RN) license.
- Hospice experience is welcome, but nurses from ICU, ER, oncology, med-surg, home health, telemetry, step-down, and palliative care are encouraged to apply.
- Strong clinical assessment and communication skills.
- Confidence working independently in the field.
- A passion for building meaningful relationships through exceptional patient care.
