What You'll Do

* Provide skilled nursing care in patients' homes

* Monitor patient conditions and report changes to the care team

* Administer medications and treatments as ordered

* Educate patients and families on care plans and disease management

* Complete accurate and timely documentation

* Collaborate with RNs, physicians, and interdisciplinary care teams

* Build trusted relationships with patients and caregivers

What You'll Need

* Current LVN/LPN license in the state of practice

* Minimum of 1 year of nursing experience in acute care or similar setting

* CPR certification through American Heart Association or American Red Cross

* Strong communication and time management skills

* Compassionate, patient-focused mindset

* Reliable transportation, valid driver's license, and current auto insurance

* Ability to frequently lift, push, pull, and support up to 50 pounds
