ResponsibilitiesResponsible for the coordination of the Long Term and Community Integration Services (LTCIS). Provide supervision, training, and guidance to assigned staff. Develop and implement Individual Service Plans (ISP) and support assigned participants. Ensure compliance with all State and Federal regulatory guidelines, including Medicaid, Licensure, and the Commission on Accreditation of Rehabilitation Facilities (CARF), as well as other funding sources and company policy and procedure.
Hire, train, and evaluate assigned staff and perform the duties as assigned by the Division Manager or designee.
Primary duties include completing and reviewing accurate and timely evaluation and assessment reports, progress reports, daily notes, incident reports, contacts, reviews, attendance, and other supporting documentation. Monitor services to ensure needs are met; supervise staff to provide comprehensive services; maintain a safe, secure, and healthy work environment and report unsafe conditions; ensure documented compliance with regulatory agencies; ensure records meet regulatory standards;
coordinate with community agencies, families, and stakeholders for program planning; complete the Orientation to Developmental Disabilities (ODD) test and competencies; coordinate and participate in Person Centered Planning (PCP) processes; maintain familiarity with behavior intervention techniques; present behavior intervention programs at county committees when needed; coordinate with support coordinators for total program services; and perform other responsibilities as assigned.

QualificationsEducation:
High school diploma or General Educational Development. Preferred:
Bachelor’s degree in Human Services or a related field.
Experience:
Three (3) years in the human services field and one year of supervisory experience.
Certification & Requirements:
Must pass State of Virginia Department of Behavioral Health and Developmental Services (DBHDS) fingerprinting and criminal background check. Must pass the ODD assessment and competencies. If driving individuals, must have a current and valid driver’s license, a good motor vehicle record (MVR), and complete online driving training within 15 days of hire.
Physical ability to support participants with personal care; ability to relate well to people at all levels; proficiency in Microsoft Office and related corporate programs such as Microsoft Teams or Zoom; problem‑solving and adaptability; excellent verbal and written communication; detail‑oriented; ability to work independently and as part of a team; knowledge of behavior intervention principles; knowledge of community resources.
Physical DemandsRegularly required to sit; reach with hands and arms; and talk or hear. Occasionally required to stand and walk. May lift or move office products and supplies, up to 50 pounds. Specific vision abilities required include close vision, distance vision, depth perception, and ability to adjust focus.
Work EnvironmentThe noise level in the work environment is usually moderate. Occasionally exposed to outside weather conditions.
