TFI Family Services is seeking a full-time Aftercare Case Manager to join our team, working to support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care.
In this role you will focus on risk management, provide crisis intervention, maintain documentation, coordinate services, and meet with families in-person to support reunification and resolve concerns.
