Summary
:
The University of North Carolina Hospitals and Clinics Department of Pharmacy's Acute Care Services Team has an opening for a highly motivated individual to provide patient care to general medicine and surgery populations.
The Clinical Pharmacist will work in a multidisciplinary environment with adult medicine and surgical services to provide comprehensive pharmaceutical care to inpatients at UNC Hospitals. Responsibilities include pharmacotherapy services, medication profile review, responding to drug information questions, order verification, problem resolution, and distributive functions as needed. The role supports the transition of care process, with a primary focus on medication reconciliation and patient counseling.
The position also involves collaboration with other Department of Pharmacy members to address needs requiring this specialization. There are opportunities to precept pharmacy students as part of UNC Eshelman School of Pharmacy collaborations. Hours are Monday through Friday, 7:00 am–3:30 pm with a weekend rotation; hours may change based on operational needs.
Education and Licensure
- Pharm
D or BS Pharmacy Degree from an ACPE-accredited school/college of pharmacy - Licensed or eligible for licensure to practice in North Carolina
- Board Certified Pharmacotherapy Specialist (BCPS) and completion of a PGY1 or PGY2 residency, or equivalent pharmacy experience, are preferred
Professional Experience

Knowledge, Skills, and Abilities
- Active pharmacist licensure with the NC Board of Pharmacy or obtainment before initiation of employment
- Receive and interpret prescriptions, review patient medication profiles/history, clarify incomplete or contraindicated prescriptions with the prescriber or other health professionals, and identify/intervene on medication-related problems.
- Analyze patient data to provide optimal drug therapy recommendations and perform clinical interventions with the provider; conduct protocol-driven services such as IV to oral dosing and stewardship activities.
- Compound and oversee preparation and dispensing of medications, including sterile products, following standard formulas and procedures.
- Comply with formulary and therapeutic substitution programs to optimize therapy while controlling costs.
- Complete required documentation in medical records, controlled substances documentation, cash reconciliation, and competency records as required.
- Counsel patients and families regarding pharmaceutical care issues as requested or required.
- Direct shift workflow and supervise pharmacy technicians, students, and/or volunteers present during the shift.
- Adhere to federal, state, local, and institutional policies and regulations.
- Identify and participate in process improvement opportunities.
- Precept students and residents and participate in training new pharmacy staff as required.
