The Pediatric Surgery RN Care Coordinator (RNCC) plays a vital role in supporting patient care and clinical operations within the Division of Pediatric Surgery and the UNCH Children’s Pediatric Surgical Clinic. As a consistent point of contact for patients and families, the RNCC provides comprehensive care coordination, including triaging clinical concerns, communicating laboratory results, processing medication refills, and managing documentation needs such as school forms and FMLA paperwork.

The RNCC also facilitates prior authorizations, coordinates with home health and DME providers, and ensures seamless transitions of care between inpatient and outpatient settings.

The RNCC participates in quality improvement initiatives, monitors patient outcomes, and contributes to process improvements that enhance clinic efficiency. Committed to professional growth, the RNCC stays current in pediatric surgical nursing practices and serves as a role model for colleagues by promoting patient‑centered, high‑quality care.

This position will primarily be out of our Raleigh Clinic but will be expected to float to Chapel Hill as needed. The role includes four 10‑hour shifts per week and may qualify for a $10,000 commitment incentive.

Summary

Provides clinical administrative nursing care coordination in support of a patient care area. Duties include one or more of the following: care coordination, discharge planning, data analysis and performance metrics, and patient/staff education.

Responsibilities
  • Assesses patient status of a specialized population. Rounds with an interdisciplinary health care team to determine patient care needs and coordinate ongoing acute and long‑term care.
  • Collaborates with the health care team to provide information and resources to facilitate patient care. Provides in‑services and formal presentations to nursing, medical, and ancillary staff on topics related to the specialized patient population. Assists staff in the development of patient education materials and performs patient teaching related to specific topics and needs.
  • Collects data related to the specialized patient population, analyzes trends, and enters data into databases and registries for benchmarking. Participates in performance improvement activities to promote quality patient care and suggests revisions to patient care protocols.
  • Coordinates patient care for post‑hospitalization and discharge planning, makes referrals to other providers, and communicates discharge needs to the interdisciplinary health care team, patient, and family. Coordinates appointments with clinics, referral sources, home care, DME providers, and other providers to ensure continuity of care.
  • Promotes own professional development through in‑services, presentations, conferences, and current literature. Role models professional nursing practice through the nursing process and professional behaviors.

Education Requirements:

  • Graduation from a state‑accredited school of professional nursing.

Licensure/Certification Requirements:

  • Licensed to practice as a Registered Nurse in the state of North Carolina.

Professional Experience Requirements:

  • Five (5) years of professional nursing experience.
