UNC Health - - Responsibilities:
Oversee screening, admissions and transfers of patients to Advanced Care at Home through comprehensive assessment, planning and coordination;
Maintain continuous contact with the Advanced Care at Home clinical command center and Administrator on Call to understand real time capacity;
Review Epic lists and EADS model lists to identify potential patients for admission or transfer;
Collaborate with attending physicians and bedside nurses to ensure appropriate patient selection and level of care;
Participate in twice daily multidisciplinary team meetings and weekly operational meetings
