Position Summary
The Pediatric Gastroenterology Nurse Coordinator is responsible for coordinating care for pediatric patients with complex gastrointestinal disorders, including inflammatory bowel disease (IBD), feeding disorders, liver disease, disorders of gut‑brain interaction, and other conditions. The nurse coordinator provides comprehensive clinical coordination, patient education, and support for complex and chronic conditions, and serves as a key liaison among patients, families, and the multidisciplinary care team.
Key Responsibilities- Coordinate care for pediatric GI patients
- Triage calls/messages with patients/families regarding symptoms and management, and draft a response
- Manage patient tracking systems (e.g., follow-up lists, infusion schedules, high‑risk patient registries) to ensure timely care
- Serve as the primary point of contact for patients, families, and providers regarding care coordination and clinical needs
- Work with scheduling team to ensure timely clinic visits, procedures, infusion appointments, hospital admissions, and annual follow‑ups
- Coordinate outside testing/procedures
- Provide patient and family education related to diagnoses, treatment plans, medications, diet, and disease management
- Coordinate care for patients receiving subcutaneous biologic therapies or infusions, including monitoring and follow‑up
- Collaborate with physicians, APPs, dietitians, and other care team members to ensure comprehensive care delivery
- Assist with prior authorizations, referrals, and coordination of specialty services
- Monitor patient outcomes and address gaps in care, including missed appointments or delayed follow‑ups
- Communicate with patients, families, and other care providers
- Maintain accurate documentation in the electronic medical record (Epic preferred)
- Support quality improvement initiatives, patient registries, and clinical program development within Pediatric GI
- Participate in division meetings, educational sessions, multidisciplinary meetings, case conferences, and care planning discussions
- Assess patient status and coordinate acute and long‑term care with interdisciplinary team
- Develop and deliver patient education and staff in‑services; document patient teaching
- Collect, analyze and report data for performance improvement; suggest protocol revisions
- Coordinate discharge planning and post‑hospitalization care, including referrals and appointments
- Engage in continuous professional development and model nursing practice
- Licensed Registered Nurse (RN) in the state of North Carolina
- Bachelor of Science in Nursing (BSN) preferred
- Strong organizational, communication, and care coordination skills
- Ability to manage complex patient populations and multiple workflows
- Proficiency with electronic medical records (Epic preferred)
- Minimum of 2–3 years of pediatric or clinical nursing experience
- Experience in pediatric gastroenterology, hepatology, and nutrition
- Experience coordinating care for infusion therapies and chronic disease management
- Familiarity with patient registries, tracking systems, and population health management
- Certification in pediatric nursing or gastroenterology (preferred but not required)
- Experience in an academic medical center or specialty clinic
This position qualifies for a $10,000 commitment incentive.
Other Information- Graduation from a state‑accredited school of professional nursing
- Licensed to practice as a Registered Nurse in the state of North Carolina
- Five (5) years of professional nursing experience
