Description

We are seeking a compassionate and organized nurse to support pediatric patients and families within a specialized outpatient care setting. This role combines patient coordination, telephone and virtual nursing support, family education, and administrative responsibilities. The ideal candidate will enjoy building relationships with children and families, helping them navigate care needs, and providing guidance throughout the patient journey. This position is well suited for a nurse who is interested in care coordination and case management responsibilities but prefers a role that is not focused exclusively on traditional case management.

Responsibilities
  • Support patients and families by assisting with rooming, coordinating paperwork, and helping families complete required questionnaires.
  • Guide families in obtaining and submitting therapy notes and other documentation needed for care.
  • Conduct nursing visits by telephone and through virtual visits using Epic.
  • Assess patient needs and help determine appropriate next steps, including whether the patient should be scheduled with a medical provider, psychologist, or another member of the care team.
  • Provide education, guidance, and ongoing support to pediatric patients and their families.
  • Assist with care coordination and administrative responsibilities.
  • Provide coverage and support for RN Coordinators as needed.
  • Collaborate closely with an interdisciplinary team to promote a positive and coordinated patient experience.
  • Use creativity, patience, and strong communication skills to effectively engage with children who have mental, behavioral, intellectual, or developmental health needs.
Patient Population

This role supports pediatric patients with a variety of developmental and behavioral health needs, including:

  • Down syndrome
  • Autism spectrum disorder
  • Intellectual and developmental disabilities

The position supports approximately 20 patients served each day.

Schedule and Work Environment
  • Choose between four 10-hour shifts or five 8-hour shifts per week.
  • No weekends or major holidays.
  • The position involves limited physical demands and does not require routine injections, IV starts, or other highly procedural nursing responsibilities.
  • The role may be a good fit for a nurse seeking lower physical demands while continuing to provide meaningful patient and family support.
Preferred Qualifications
  • Pediatric nursing experience is strongly preferred.
  • Spanish-speaking or bilingual experience is a plus.
  • Experience supporting patients with mental or behavioral health needs is preferred.
  • Experience working with children with autism, Down syndrome, or intellectual and developmental disabilities is highly valued.

Summary

Provides competent clinical nursing care consistent with professional standards. Reporting and accountable to the Nurse Manager/Director, the Clinical Nurse is responsible for coordinating and delivering patient care utilizing the nursing process in a hospital setting.

Responsibilities
  • Education
    - Participates in identifying and meeting learning needs of self. Attends education programs based on identified learning needs. With assistance, uses patient education materials relevant to patient population. Assesses readiness to learn of the patient/family/caregivers. Assesses developmental level of patient and factors affecting ability to learn.
  • Evaluation of Care
    - Identifies expected patient outcomes. Seeks guidance as needed in revision of plan of care. Participates in discussion with members of the interdisciplinary team in evaluation of patient care. Communicates relevant information to promote continuity of care.
  • Implementation
    - Demonstrates competence in care of patients with complex problems, including population-appropriate physical, psychosocial, educational and safety aspects of care. Performs bedside point of care testing as required for patient care plan. Utilizes healthcare organization and nursing standards, policies and procedures in delivery of care. Organizes and prioritizes care according to patient/family needs. Consults with appropriate resources in a timely fashion regarding patients with complex care issues, unusual teaching needs and/or those at high risk for discharge planning.
  • Leadership
    - With assistance, develops goals to promote professional growth or minimize limitations. Achieves goals and objectives within identified time frame or renegotiates with supervisor. Is knowledgeable about activities which facilitate intra/interdepartmental collaboration. Participates in development and achievement of unit goals and performance improvement activities.
