Description:

The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction. It promotes continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning. The Care Manager must be a highly organized professional with great attention to detail.

Adaptability to frequent change is essential. Compliance with regulatory and departmental guidelines and policies is required. Essential duties include identifying cases and prioritizing daily work lists to prioritize patients and identify new admissions. Conduct and document assessment and a plan of care in accordance with departmental guidelines. Participate in daily care management meetings as established in protocols. Consult with social workers as per established criteria.

Communicate with the Care Management Assistant to share priorities when indicated. Attend and actively participate in relevant meetings for assigned units to provide and receive information on patient progression. Alert the care team to concerns impacting anticipated discharge and any care assisting with discharge readiness. Modify discharge plans based on information shared during meetings. Assist in identifying expected discharge dates.

Complete necessary follow-ups as appropriate. Attend weekly complex care meetings. Present on patients during these meetings and collaborate with the team to address issues with complex patients. Continuously monitor cases and follow up on all action items. Proactively identify high-risk cases needing escalation to meetings if not already scheduled. Discuss with appropriate members of the multidisciplinary team regarding barriers to discharge and psychosocial concerns impacting care progression or readmission risk.

Coordinate family meetings as necessary to support care progression. Provide education on community resources and appropriate support groups to patients, families, and care teams. Educate and coordinate referrals to community resources and post-acute providers as needed. Communicate medical milestones for transition with patients and families. Assess discharge plans continuously to determine post-discharge needs and communicate them effectively to patients and families.

Identify required authorizations for post-discharge services and refer to the appropriate providers. Participate in medication resource management for patients as needed. Verify the patient's understanding and agreement of the discharge plan. Refer administrative tasks to the Care Management Assistant. Consult social workers and/or utilization managers as per departmental protocol.
