Care Manager

The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction. This role promotes continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning. The Care Manager must be a highly organized professional with great attention to detail.

The role requires adaptability to frequent change and compliance with regulatory and departmental guidelines and policies.

Essential Duties:

  • Identify cases and prioritize daily work lists to manage patients and new admissions.
  • Conduct and document assessment and a plan of care per departmental guidelines.
  • Participate in daily care management meetings according to established protocols.
  • Consult with applicable team members per established criteria.
  • Communicate with the Care Management Assistant to share priorities as needed.
  • Attend and actively participate in meetings for assigned units to provide and receive patient information.
  • Alert the care team to concerns that could impact anticipated patient discharge and any related care to assist with discharge readiness.
  • Modify discharge plans based on information shared in meetings.
  • Assist with the identification of the expected discharge date.
  • Complete follow-ups as appropriate after meetings.
  • Attend weekly complex care meetings and present on patients while collaborating to solve issues with complex cases.
  • Proactively identify high-risk cases that require escalation.
  • Discuss barriers to discharge and psychosocial concerns with the multidisciplinary team.
  • Coordinate family meetings to support care progression as necessary.
  • Provide education on community resources, support groups, and other appropriate resources to patients, families, and care teams.
  • Educate and coordinate referrals to community resources and post-acute providers as needed.
  • Communicate medical milestones for transition with patients and families.
  • Identify patients with barriers to discharge based on ongoing assessments.
  • Monitor all observation patients to ensure appropriate progression of care.
  • Assess discharge plans for post-discharge needs and communicate these with patients and families.
  • Identify required authorizations for post-discharge services and refer to appropriate providers.
  • Participate in medication resource management for patients as needed.
  • Verify patient understanding and agreement of discharge plans.
  • Refer administrative tasks to the Care Management Assistant.
  • Consult with relevant team members per established departmental protocol.

Experience:

2+ years of Care Management or Case Management experience in an acute care setting required. License:
Licensed to practice as a Registered Nurse in the state of North Carolina.
