TheUNC Hospitals Cardiac Rehab Programis a well-established,AACVPR-certified program located within theUNC Wellness Center at Meadowmont Village in Chapel Hill, NC. Our program provides a unique, supportive environment where patients participate in cardiac and pulmonary rehabilitation in a bright, welcoming clinical fitness setting focused on improving health, function, and quality of life.

Clinical Nurse II
- Day Shiftto join our collaborative, multidisciplinary team. In this role, you will provide comprehensive nursing care and clinical monitoring for patients participating in our cardiac and pulmonary rehabilitation programs. You will work closely with physicians, exercise physiologists, and other members of the healthcare team to promote patient safety, support individualized treatment goals, and optimize patient outcomes.
- Provide nursing care and clinical oversight for patients participating in cardiac and pulmonary rehabilitation.
- Monitor patients and telemetry during rehabilitation sessions, recognizing and responding to changes in clinical status.
- Perform comprehensive initial nursing assessments and ongoing assessments as indicated.
- Develop, implement, and update individualized treatment plans in collaboration with the multidisciplinary team.
- Communicate patient concerns, changes in condition, and other clinical findings to the supervising physician and appropriate members of the care team.
- Respond appropriately to medical emergencies and identified health concerns during rehabilitation sessions.
- Provide patient education, counseling, and ongoing support related to disease management, risk-factor modification, exercise, and overall wellness.
- Promote a safe, supportive, and patient-centered rehabilitation environment.
- Collaborate with patients, families, physicians, and multidisciplinary team members to support individualized goals and optimal outcomes.
- 1-3 years of cardiovascular nursing experience in an inpatient setting preferred; outpatient cardiovascular nursing experience will also be considered.
- OR 3 years of experience in a cardiopulmonary rehabilitation setting.
- Strong assessment, communication, critical-thinking, and patient education skills.
- Ability to work effectively in a collaborative, multidisciplinary environment.
- Experience with telemetry monitoring and recognition of cardiac rhythm changes preferred.

SummaryProvides competent clinical nursing care consistent with professional standards. Reporting and accountable to the Nurse Manager/Director, the Clinical Nurse is responsible for coordinating and delivering patient care utilizing the nursing process in a hospital setting.
Responsibilities- 1. Education
- Participates in identifying and meeting learning needs of self. Attends education programs based on identified learning needs. With assistance, uses patient education materials relevant to patient population. Assesses readiness to learn of the patient/family/caregivers. Assesses developmental level of patient and factors affecting ability to learn. - 2. Evaluation of Care
- Identifies expected patient outcomes. Seeks guidance as needed in revision of plan of care. Participates in discussion with members of the interdisciplinary team in evaluation of patient care. Communicates relevant information to promote continuity of care. - 3. Implementation
- Demonstrates competence in care of patients with complex problems, including population-appropriate physical, psychosocial, educational and safety aspects of care. Performs bedside point of care testing as required for patient care plan. Utilizes healthcare organization and nursing standards, policies and procedures in delivery of care.
