Advocate Health in North Carolina is seeking a full-time Case Manager RN to coordinate patient care across the care continuum. You will work 40 hours weekly, Monday–Friday 8:00am-4:30pm with occasional weekend coverage, and may operate within a provider office or telephonically with a care team.
Responsibilities include collaborating with multidisciplinary teams, communicating with patients and families, and ensuring timely completion of documentation for MCO and regulatory requirements.
