Position SummaryThis position will serve as a Care Manager for the Center for Excellence in Community Mental Health ( CECMH ). The care management team at the CECMH offers robust and effective service models to address the comprehensive health needs of eligible beneficiaries through collaboration with insurance providers, UNC providers, and community partners.
This position will provide care management services to an assigned panel of adults with serious mental illness and other behavioral health needs. The Care Manager will engage with assigned beneficiaries through frequent contact based on client need and acuity and will be responsible for ensuring referrals to psychiatry/medication…
