Key Responsibilities
  • Serve as the primary liaison between inpatient psychiatric hospitals and the Care Management Entity (CME) to ensure continuity of care for youth transitioning from higher levels of psychiatric care to community-based services.
  • Establish relationships with local hospital systems to better support referrals and discharge planning. Through this relationship development work to visit local hospital departments at minimum once per month to further relationship and streamlining of referrals.
  • Designate and sustain a direct point-of-contact within each hospital’s social work or discharge planning department to bypass general intake or referral processes.
  • Maintain a consistent physical and/or virtual presence within inpatient psychiatric units to ensure real-time awareness of admissions, treatment planning, and anticipated discharges.
  • Develop and maintain collaborative relationships with hospital social workers, discharge planners, nursing staff, psychiatrists, and community providers.
  • Participate in psychiatric discharge planning to ensure CME prior to discharge, with emphasis on safe and timely transitions.
  • Coordinate warm handoffs among inpatient providers, CME staff, outpatient providers, youth, and families by working diligently to protect the 72-hour post discharge window by monitoring service linkage, medication access, and follow-up appointments.
  • Identify barriers to discharge or stabilization and present CME‑aligned solutions to hospital teams.
  • Manually enter real‑time admission and discharge data into CME tracking systems when Clini Sync notifications are unavailable or delayed.
  • Provide education to hospital partners regarding CME services and community-based resources.
  • Maintain accurate, timely, and confidential documentation in accordance with Ohio Administrative Code, Medicaid requirements, CME standards, and agency policies.
  • Maintain effective communication for all cases assigned with all partners involved through staffing, weekly rounds, or child and family team meetings.
  • Assist to track statistics and outcomes related to transition coordination, discharge involvement, and post‑discharge continuity.
