The Registered Nurse Case Manager (RNCM) ix Hospice leads each patient’s hospice care team, developing the Plan of Care in collaboration with the Hospice Medical Director, patient, and family.
You will provide day-to-day nursing care, coordinate with physicians and the interdisciplinary team, educate families, ensure HIPAA compliance, and participate in on-call visits as needed.
This role emphasizes comfort, quality of life, and compassionate support in home and facility settings.
