Licensed Practical Nurse

Shenandoah Center in Clarksburg, WV is seeking a Licensed Practical Nurse to provide compassionate bedside care for residents in our skilled nursing and rehabilitation facility. The LPN will administer medications and treatments, monitor vital signs, document in the EMR, and support care plans in collaboration with RNs, therapists, and physicians. This resident-focused, team-oriented role offers the chance to build strong relationships, grow clinical skills, and contribute to a culture of respect, safety, and continuous improvement in long-term care and rehab.

Responsibilities

• Provide direct bedside nursing care to residents under RN and provider direction
• Administer medications and treatments per physician orders and facility policy
• Monitor and document vital signs, resident status, and responses to treatment in EMRSupport admission, transfer, and discharge processes, including assessments and education
• Assist with wound care, catheter care, and basic therapeutic treatments per scope of practice
• Collaborate with RNs, therapists, and physicians on individualized care plans
• Assist residents with ADLs while promoting dignity, safety, and comfort
• Recognize and promptly report changes in condition or adverse events
• Follow infection control, safety, and regulatory standards at all times
• Educate residents and families on care plans, medications, and health maintenance
