Licensed Practical Nurse (LPN) – 2nd Shift

We are hiring for a Licensed Practical Nurse (LPN) in our 2nd Shift at Hunter Ridge – Intermediate Care Facility (ICF) in Humboldt, TN. This is a full‑time, on‑site position with a schedule of Monday – Friday, 4pm – 12am. Compensation is $30.42 per hour with no sign‑on bonus or relocation package offered.

• Provide holistic nursing care to individuals with intellectual and developmental disabilities.
• Assist the Registered Nurse in delivering quality nursing services.
• Provide health‑related training to unlicensed personnel.
• Administer medications and treatments or assist in medication administration.
• Assist with ensuring preventative and routine medical appointments are scheduled.
• Conduct medication‑centered assessments and collect health data.
• Observe for and report any abnormal health findings to the physician.
• Communicate with physicians, pharmacies, medical suppliers, and the interdisciplinary team.
• Advocate for residents and assist with developing community supports to promote optimal health.

• Graduation from an accredited licensed nurse program.
• Current nursing license with no restrictions.
• Current CPR certification or eligibility to obtain certification.
• Effective communication skills with interdisciplinary team members, residents and families.
• Ability to work independently, be organized and prioritize tasks.

• Regularly lift up to 20 pounds, and at least 40 pounds as needed.
• Pull a minimum of 25 pounds.
• Perform squatting, kneeling, crawling, crouching, climbing, stooping, standing and walking as required.
• Ability to demonstrate proficiency in CPR from the floor level.

• Criminal background check (OIG exclusions database, Child Abuse Registry, Offenders Against Individuals with Developmental Disabilities).
• Drug testing.
• Education verification and credentialing.
• Proof of employment history or references (if required).
