Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The visit lasts approximately 45 minutes, including a medication and medical history review, a physical evaluation, and, when ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. You will use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medication, order lab tests, or alter the current treatment regimen.

• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products such as spirometry, DEE, bone density screening, and more.

Your primary workplace is the private residence of health‑plan members. Diverse home environments may contain pets, secondhand smoke, and airborne allergens. Candidates must be able to perform duties safely and comfortably in these settings.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).

The typical pay range for this role is $37.66 – $81.11 per hour, depending on experience, education, geography, and other factors.
