About the Role
Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations to improve health outcomes for individuals.
Job HighlightsThe typical visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, when ordered by the member’s health plan, additional diagnostic tests such as diabetic eye exams, spirometry, or other clinical screenings.
Key Responsibilities- Conduct in‑home visits, engaging with patients to assess health status and answer questions.
- Complete medication and medical history review, perform physical evaluation, and administer diagnostic tests as ordered.
- Use an iPad and an intuitive clinical workflow for each evaluation.
- Provide services such as spirometry, diabetic eye exams, bone density screening, and other preventive services.
- Travel locally and within licensed states as needed; conduct virtual visits when required.
- Build trusted relationships and connect patients with appropriate care.
- Active, unrestricted license(s) in the coverage area(s) of practice.
- Board certification (required).
- Multi‑state license(s) (preferred).
- Minimum of 29.5 hours per week in a part‑time capacity.
$37.66 – $81.11 per hour (or equivalent full‑time salary). Salary will be based on experience, education, geography, and other factors.
