Licensed Practical Nurse

CVS Health is seeking a compassionate Licensed Practical Nurse to deliver high-quality, patient-centered care in a collaborative healthcare environment. The LPN will perform direct patient care, administer medications, support care plans, and communicate clinical information to providers and patients. Responsibilities include taking vitals, wound care, documentation in electronic health records, assisting with care coordination, and educating patients on treatments and medications.

You'll work closely with an interdisciplinary team, uphold safety and quality standards, and contribute to CVS Health's mission of helping people on their path to better health.

Responsibilities

• Provide direct patient care and support daily clinical activities
• Administer medications and treatments per provider orders
• Obtain and document vital signs, histories, and patient assessments
• Maintain accurate, timely electronic health records (EHR)
• Assist with wound care, specimen collection, and basic procedures
• Educate patients on medications, treatments, and self-care
• Coordinate with RNs, providers, and care teams for continuity of care
• Follow infection control, safety, and quality standards
• Support patient intake, discharge, and follow-up processes
• Contribute to a respectful, inclusive, patient-centered environment
