The Per Diem Postpartum & Lactation Home Visit Clinician provides in-home postpartum assessment, lactation support, and education to postpartum mothers and newborns following hospital discharge. This role supports maternal recovery, infant health, and successful feeding while identifying early complications and facilitating timely referrals. The clinician works independently in the field while collaborating closely with the interdisciplinary care team.

Key Responsibilities Clinical Care
  • Perform comprehensive postpartum and newborn assessments during home visits
  • Monitor maternal recovery, including vital signs, bleeding, incision/wound healing, pain, and overall well-being
  • Assess newborn health, including feeding effectiveness, (weight trends, jaundice risk) and general condition
  • Identify signs of maternal or newborn complications and initiate appropriate interventions or referrals
Lactation Support
  • Provide hands-on breastfeeding support and lactation education in the home setting
  • Assess latch, milk transfer, supply concerns, and feeding challenges
  • Educate families on pumping, milk storage, supplementation when indicated, and infant feeding cues
  • Support families using breastfed, expressed milk, combination, or formula feeding approaches
  • Refer to midwives or specialists when needs exceed scope of practice
Education & Family Support
  • Educate families on postpartum recovery, newborn care, safe sleep, and infant safety
  • Screen for postpartum mood and anxiety disorders and provide appropriate resources
  • Offer emotional support and guidance during the transition to home care
  • Answer patient and family questions and reinforce discharge instructions
Care Coordination & Documentation
  • Accurately document all assessments, interventions, lactation support, and education provided
  • Communicate findings and concerns to on call clinician
  • Coordinate follow-up care and community resources as appropriate
Compliance & Professional Practice
  • Adhere to organizational policies, clinical protocols, and infection control standards
  • Maintain patient confidentiality and comply with HIPAA regulations
  • Participate in required training, competency validation, and quality initiatives
Qualifications Required
  • Active, unrestricted registered nurse (RN), certified nurse midwife (CNM), or Licensed Midwife (LM) license in the state of CA
  • Ideally 1-2 years of experience in postpartum, maternal-child health, L&D, newborn care, or lactation support
  • Demonstrated experience providing breastfeeding and lactation education
  • Current BLS (CPR) certification
  • Valid driver's license and reliable transportation
  • Strong clinical assessment, communication, and patient education skills
Preferred
  • Lactation certification (CLC, IBCLC, or equivalent)
  • Home health or community-based nursing experience
  • Experience with electronic health records (EHR)
  • Bilingual or multilingual abilities relevant to the patient population
Schedule & Work Environment
  • Per diem, as-needed scheduling based on patient volume
  • Field-based role providing care in patients' homes within assigned service area
Physical Requirements
  • Ability to travel between patient homes
  • Ability to stand, walk, bend, and lift clinical supplies or equipment as required
  • Ability to perform nursing assessments in varied home environments
Compensation
  • Competitive per-visit or hourly pay based on experience
  • Mileage reimbursement
  • This per diem position does not include benefits
