Description

Concordance’s reentry program is designed to dramatically reduce recidivism by helping individuals with justice involvement lead meaningful, abundant, and joyful lives. Our holistic, evidence‑based model offers a wide range of services – therapy, case management, yoga, behavioral health and substance use recovery groups, spiritual care, life skills, medical care, education, and job readiness. Each participant is formally supported by our Care Team for a full year, ensuring stability and long‑term success.

Position Overview: The Case Manager serves as a trusted guide, advocate, and accountability partner for participants by monitoring progress and helping participants navigate through obstacles. Case Managers facilitate both individual and group sessions with participants and serve as the designated leader of participants’ care teams. The Case Manager collaborates with the care team to ensure participants are obtaining quality support as they advance through the program model.

Key Responsibilities
  • Facilitate participant programming: conduct impactful 1:1 and group sessions, developing a comprehensive understanding of every participant on caseload
  • Facilitate group sessions focused on life skills such as money management, healthy sexuality, hygiene, self‑care, and refusal skills
  • Provide meaningful 1:1 sessions with participants to explore areas for growth and encouragement to persevere, and ensure treatment goals are met
  • Uphold the quality and fidelity of Concordance re‑entry model curriculum when delivering 1:1 sessions and groups
  • Deliver participant Reflections when necessary, in a timely and trauma‑informed manner
  • Connect participants to community resources such as housing, food, healthcare
  • Support Center‑wide events
  • Perform other tasks as needed
Communicate with multi‑disciplinary care team
  • Document sessions (for individual and groups), including noteworthy participant interactions in electronic record system
  • Document Reflections using standard forms
  • Collaborate with the Care Team, including the Career Coach, and Therapist assigned to each participant, providing input on progress to ensure consistency in instruction/learning pre‑to‑post release and alignment with participant goals
  • Communicate daily with Center Coordinator regarding participant scheduling needs
  • Participate in internal meetings, community partnership meetings and trainings

Required Education and Credentials
  • Bachelor’s degree in social service‑related field (social work, psychology, counseling, criminal justice, sociology, or similar)
  • CPR/First Aid, preferred, or able to complete within first 90 days
Skills and Competencies
  • 1–3 years minimum experience implementing case management services and documenting interactions
  • Prior experience with justice‑involved adults or other vulnerable populations
  • Proficient in utilizing computer‑based programs to document, locate resources, support participants in applications, and upload forms
  • Aware of impact of trauma and substance misuse
  • Knowledgeable about community resources
Physical Requirements

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. This person must be able to sit or stand at a personal computer for typing and reading.
